In this paper, we analyse the most important documents establishing the criteria for the treatment and exclusion of COVID-19 patients, especially in regard to the giving of respiratory support, in Italy and Spain. These documents reflect a tension that stems from limited healthcare resources which are insufficient to save lives that, under normal conditions, could have been saved, or at least could have received the best possible treatment. First, we analyse the healthcare systems of these two countries before the spread (...) of the virus, both of which have seen decreases in the number of intensive care beds and have been marked by financial cuts during the last ten years. It is a fact that a greater number of people, especially those over 70 years of age, have been left without respiratory support treatment, and therefore, there have been a greater number of deaths. It is also a fact that there has been a higher infection rate among healthcare professionals due to the delay in the management of protective measures and the inability to provide adequate care for those in nursing homes, as recognised by WHO. In the context of this health emergency, healthcare professionals have suffered a real ‘moral distress’ because, knowing first-hand the causes of the limitation of resources, they have had to put triage protocols into practice. Finally, we set forth a series of concrete ethical proposals with which to face the successive waves of COVID-19 infection, as well as other future pandemics. (shrink)

The two authors of this paper have diametrically opposed views of the prevalence and strength of adaptation in nature. Hendry believes that adaptation can be seen almost everywhere and that evidence for it is overwhelming and ubiquitous. Gonzalez believes that adaptation is uncommon and that evidence for it is ambiguous at best. Neither author is certifiable to the knowledge of the other, leaving each to wonder where the other has his head buried. Extensive argument has revealed that each author thinks (...) his own view is amply supported by both theory and empirical evidence. Further reflection has revealed that the differences in opinion may start with the different disciplines in which we work: evolutionary ecology for Hendry and community ecology for Gonzalez. In the present paper, we each present devastating evidence supporting our own position and thus refuting that of the other. We then identify the critical differences that led to such opposing views. We close by combining our two perspectives into a common framework based on the adaptive landscape, and thereby suggest means by which to assess the prevalence and strength of adaptation. (shrink)

We present a proof of the equivalence between two deductive systems for constructive necessity, namely an axiomatic characterisation inspired by Hakli and Negri's system of derivations from assumptions for modal logic , a Hilbert-style formalism designed to ensure the validity of the deduction theorem, and the judgmental reconstruction given by Pfenning and Davies by means of a natural deduction approach that makes a distinction between valid and true formulae, constructively. Both systems and the proof of their equivalence are formally verified (...) using the state-of-the-art proof assistant Coq. The proof approach taken throughout the paper unveils the use of some alternative proof methods that allow for a smooth transition from the high-level mathematical proofs to their mechanised counterparts. This book paints a comprehensive portrait of Mexico’s system of assisted reproduction first from a historical perspective, then from a more contemporary viewpoint. Based on a detailed analysis of books and articles published between the 1950s and 1980s, the first section tells the story of how the epistemic, normative, and material infrastructure of the assisted reproduction system was built. It traces the professionalization process of assisted reproduction as a medical field and the establishment of its professional association. Drawing on ethnographic (...) material, the second part looks at how this system developed and flourished from the 1980s up to 2010, its commercialization process, how the expansion of reproductive services took place, and the messages regarding reproductive technologies that circulated within a wide discursive landscape. Given its scope and methods, this book will appeal to scholars interested in science and technology studies, reproduction studies, history of medicine, medical anthropology, and sociology. (shrink)

This article has two objectives: the first, to analyze the professionalization process of assisted reproduction (AR) in order to see how AR is consolidating into an independent field within medicine, and the second, to see how AR arrived and was assimilated into Mexican culture. As opposed to other projects that have traced back the story of a particular specialty to see how it emerged as such, this article looks at an ongoing process: specialization in action. By analyzing the data collected (...) through three years of multisited ethnography using tools offered by the sociology of professions and the history of medical specialization, I identify specific moments in which AR is gaining cognitive, normative, and evaluative consolidation, structure, and independence. This leads me to suggest that AR might separate from gynecology and become an independent specialty within medicine. I offer examples to support this argument and show how AR has developed into a biomedical service, a professional biomedical field, and a biomedical business. (shrink)

Directional deep brain stimulation contacts provide greater spatial flexibility for therapy than traditional ring-shaped electrodes, but little is known about longitudinal changes of impedance and orientation. We measured monopolar and bipolar impedance of DBS contacts in 31 patients who underwent unilateral subthalamic nucleus deep brain stimulation as part of a randomized study. At different follow-up visits, patients were assigned new stimulation configurations and impedance was measured. Additionally, we measured the orientation of the directional lead during surgery, immediately after surgery, and (...) 1 year later. Here we contrast impedances in directional versus ring contacts with respect to local anatomy, active stimulation contact, and over time. Directional contacts display larger impedances than ring contacts. Impedances generally increase slightly over the first year of therapy, save for a transient decrease immediately post-surgery under general anesthesia during pulse generator placement. Local impedances decrease at active stimulation sites, and contacts in closest proximity to internal capsule display higher impedances than other anatomic sites. DBS leads rotate slightly in the immediate postoperative period but otherwise remain stable over the following year. These data provide useful information for setting clinical stimulation parameters over time. (shrink)

Equational reasoning arises in many areas of mathematics and computer science. It is a cornerstone of algebraic reasoning and results essential in tasks of specification and verification in functional programming, where a program is mainly a set of equations. The usual manipulation of identities while conducting informal proofs obviates many intermediate steps that are neccesary while developing them using a formal system, such as the equationally complete Birkhoff calculus ${\mathcal{B}}$. This deductive system does not fit in the common manner of (...) doing mathematical proofs, and it is not compatible with the mechanisms of proof assistants. The aim of this work is to provide a deductive system ${\mathcal{B}}^{\textrm{GOAL}}$ for equality, equivalent to ${\mathcal{B}}$ but suitable for constructing equational proofs in a backward fashion. This feature makes it adequate for interactive proof-search in the approach of proof assistants. This will be achieved by turning ${\mathcal{B}}^{\textrm{GOAL}}$ into a transition system of formal tactics in the style of Edinburgh LCF, such transformation allows us to give a direct formal definition of backward proof in equational logic. The main topics of this thesis are cardinal invariants, P -points and MAD families. Cardinal invariants of the continuum are cardinal numbers that are bigger than $\aleph _{0}$ and smaller or equal than $\mathfrak {c}.$ Of course, they are only interesting when they have some combinatorial or topological definition. An almost disjoint family is a family of infinite subsets of $\omega $ such that the intersection of any two of its elements is finite. A MAD family is a maximal almost (...) disjoint family. An ultrafilter $\mathcal {U}$ on $\omega $ is called a P-point if every countable $\mathcal {B\subseteq U}$ there is $X\in $ $\mathcal {U}$ such that $X\setminus B$ is finite for every $B\in \mathcal {B}.$ This kind of ultrafilters has been extensively studied, however there is still a large number of open questions about them.In the preliminaries we recall the principal properties of filters, ultrafilters, ideals, MAD families and cardinal invariants of the continuum. We present the construction of Shelah, Mildenberger, Raghavan, and Steprāns of a completely separable MAD family under $\mathfrak {s\leq a}.$ None of the results in this chapter are due to the author.The second chapter is dedicated to a principle of Sierpiński. The principle $\left $ of Sierpiński is the following statement: There is a family of functions $\left \{ \varphi _{n}:\omega _{1}\longrightarrow \omega _{1}\mid n\in \omega \right \} $ such that for every $I\in \left [ \omega _{1}\right ] ^{\omega _{1}}$ there is $n\in \omega $ for which $\varphi _{n}\left [ I\right ] =\omega _{1}.$ This principle was recently studied by Arnie Miller. He showed that this principle is equivalent to the following statement: There is a set $X=\left \{ f_{\alpha }\mid \alpha \alpha $ then $f_{\beta }\cap g$ is infinite. Miller showed that the principle of Sierpiński implies that non $\left =\omega _{1}.$ He asked if the converse was true, i.e., does non $\left =\omega _{1}$ imply the principle $\left $ of Sierpiński? We answer his question affirmatively. In other words, we show that non $\left =\omega _{1}$ is enough to construct an $\mathcal {IE}$ -Luzin set. It is not hard to see that the $\mathcal {IE}$ -Luzin set we constructed is meager. This is no coincidence, because with the aid of an inaccessible cardinal, we construct a model where non $\left =\omega _{1}$ and every $\mathcal {IE}$ -Luzin set is meager.The third chapter is dedicated to a conjecture of Hrušák. Michael Hrušák conjectured the following: Every Borel cardinal invariant is either at most non $\left $ or at least cov $\left $. Although the veracity of this conjecture is still an open problem, we were able to obtain some partial results: The conjecture is false for “Borel invariants of $\omega _{1}^{\omega }$ ” nevertheless, it is true for a large class of definable invariants. This is part of a joint work with Michael Hrušák and Jindřich Zapletal.In the fourth chapter we present a survey on destructibility of ideals and MAD families. We prove several classic theorems, but we also prove some new results. For example, we show that every almost disjoint family of size less than $\mathfrak {c}$ can be extended to a Cohen indestructible MAD family is equivalent to $\mathfrak {b=c}$. A MAD family $\mathcal {A}$ is Shelah–Steprāns if for every $X\subseteq \left [ \omega \right ] ^{<\omega }\setminus \left \{ \emptyset \right \} $ either there is $A\in \mathcal {I}\left $ such that $s\cap A\neq \emptyset $ for every $s\in X$ or there is $B\in \mathcal {I}\left $ that contains infinitely many elements of X $ denotes the ideal generated by $\mathcal {A}$ ). This concept was introduced by Raghavan which is connected to the notion of “strongly separable” introduced by Shelah and Steprāns. We prove that Shelah–Steprāns MAD families have very strong indestructibility properties: Shelah–Steprāns MAD families are indestructible for “many” definable forcings that does not add dominating reals. According to the author’s best knowledge, this is the strongest notion that has been considered in the literature so far. In spite of their strong indestructibility, Shelah–Steprāns MAD families can be destroyed by a ccc forcing that does not add unsplit or dominating reals. We also consider some strong combinatorial properties of MAD families and show the relationships between them.The fifth chapter is one of the most important chapters in the thesis. A MAD family $\mathcal {A}$ is called $+$ -Ramsey if every tree that branches into $\mathcal {I}\left $ -positive sets has an $\mathcal {I}\left $ -positive branch. Michael Hrušák’s first published question is the following: Is there a $+$ -Ramsey MAD family? It was previously known that such families can consistently exist. However, there was no construction of such families using only the axioms of ZFC. We solve this problem by constructing such a family without any extra assumptions.In the fourth and fifth chapters, we introduce several notions of MAD families, in the sixth chapter we prove several implications and non implications between them. We construct several MAD families with different properties.In the seventh chapter we build models without P -points. We show that there are no P -points after adding Silver reals either iteratively or by the side by side product. These results have some important consequences: The first one is that is its possible to get rid of P -points using only definable forcings. This answers a question of Michael Hrušák. We can also use our results to build models with no P -points and with arbitrarily large continuum, which was also an open question. According to Plutarch (Sert. 7.1–2) and Sallust (Hist. 1.83–4), a certain P. Calpurnius Lanarius killed L. Livius Salinator, a member of Sertorius’ staff in charge of military operations in the Pyrenees, in the early stages of the Sertorian War (82–72 b.c.). Through the analysis of the verb δολοφονέω in Plutarch's and St. Jerome's use of Sallust's Histories, this article seeks to demonstrate that Lanarius was an exile of the Sullan regime who treacherously assassinated his superior Salinator. In this article, we present results from a literature review of intrinsic, instrumental, and relational values of nature conducted for the Intergovernmental Science-Policy Platform on Biodiversity and Ecosystem Services, as part of the Methodological Assessment of the Diverse Values and Valuations of Nature. We identify the most frequently recurring meanings in the heterogeneous use of different value types and their association with worldviews and other key concepts. From frequent uses, we determine a core meaning for each value type, which is (...) sufficiently inclusive to serve as an umbrella over different understandings in the literature and specific enough to help highlight its difference from the other types of values. Finally, we discuss convergences, overlapping areas, and fuzzy boundaries between different value types to facilitate dialogue, reduce misunderstandings, and improve the methods for valuation of nature's contributions to people, including ecosystem services, to inform policy and direct future research. © 2023 The Author(s). I argue that if there are nondisabled reasons to believe p, then there cannot be nondisabled reasons to believe something incompatible with p. I first defend a restricted version of the view, which applies only to situations where the relevant agent has complete evidence. Then, I argue for a generalized version of the view, which holds regardless of the agent's evidence. As a related result, I show that, given plausible assumptions, there cannot be nondisabled reasons to believe something false.

According to logical non-necessitarianism, every inference may fail in some situation. In his defense of logical monism, Graham Priest has put forward an argument against non-necessitarianism based on the meaning of connectives. According to him, as long as the meanings of connectives are fixed, some inferences have to hold in all situations. Hence, in order to accept the non-necessitarianist thesis one would have to dispose arbitrarily of those meanings. The training of healthcare professionals goes beyond technical skills, requiring the development of humanistic competencies to balance the technical and the human aspects. Bioethics, essential in health programs, addresses juridical-moral conflicts arising from the interaction between social values and science. The inclusion of bioethics in curricula is crucial to prepare healthcare professionals for contemporary ethical dilemmas. This study aims to highlight the perception of bioethical and professional deontology knowledge among Health Degree students, providing a critical foundation to strengthen ethical education (...) and address emerging ethical challenges in their professional practice. A total of 150 students in physiotherapy, nursing, and psychology at the European University of Valencia were evaluated through an Ad-Hoc survey. The scale measured bioethical knowledge with 19 Likert-scale items. Internal consistency was adequate (α = 0.872). Statistical analysis revealed significant differences in bioethical knowledge among the studied degrees (p<0.05). First-year students scored higher on “The end justifies the means” and “I know the principle of beneficence. ” First and third-year students valued equality in patient treatment. In applied ethics, fourth-year students stood out. (shrink)

It is necessary to reconsider P. M. S. Hacker’s assessment of Kant and Wittgenstein’s philosophical affinities and the question concerning Wittgenstein’s alleged use of “transcendental arguments”. First, Alfred Norman’s reading of the Tractatus Logico-Philosophicus as a thought experiment receives revision to develop a view of the Critique of Pure Reason as a large-scale thought experiment that shares important logical features with the Tractatus. Then the question is addressed whether the middle Wittgenstein and the pre-critical Kant employed any thought experiments that (...) could be equally characterised as “transcendental arguments”. A rational reconstruction of both thinkers’ arguments is carried out in the light of the contemporary literature on thought experimenting. The novelty and relevance of this approach is the emphasis laid upon a largely neglected affinity between Kant and Wittgenstein, namely the systematic use of thought experiments in their epistemological pursuits. The conclusions are: i) Wittgenstein’s and Kant’s magna opera can be seen as philosophical thought experiments that attempt to try out the limits of language and the limits of possible experience respectively; ii) Both philosophers developed arguments that can be designated as transcendental if only from a methodological standpoint; and iii) some key arguments put forward by the middle Wittgenstein in the determination of the structure of visual space could be characterised better as thought experiments than transcendental arguments. Resumen Cuando el 6 de agosto de 1945 los científicos del proyecto atómico alemán dirigido por el físico Werner Heisenberg escucharon desde su encierro en Farm Hall la noticia de que Estados Unidos había arrojado una bomba nuclear que había destruido Hiroshima, comenzó la construcción de la historia: la versión que Heisenberg ofreció en su autobiografía intelectual fue que sabía que el proyecto era inviable a corto y medio plazo y, por tanto, no había riesgo en asumir la investigación. Siguiendo (...) las tesis de filosofía de la historia de Benjamin, pasaré el cepillo a contrapelo a esta construcción de la historia de la ciencia para sacar a la luz varias razones que la desmontan. Para ello recurriré a la filosofía de la ciencia de Popper, Kuhn y Lakatos y a la critica de Marcuse a la racionalidad tecnológica. La finalidad es triple: cuestionar la visión de la ciencia sostenida por Heisenberg, mostrar la influencia que ejercen los grandes proyectos científico - tecnológicos sobre el modo de pensar y actuar de los individuos y salvar los dos abismos denunciados por el físico y novelista C. P. Snow, uno entre ciencias y humanidades y otro entre quienes poseen conocimiento especializado y quienes no.When, on August 6, 1945, the scientists ofthe German atomic project led by physicist Werner Heisenberg heard the news from their confinement in Farm Hall, England, that the United States had dropped a nuclear bomb that destroyed Hiroshima, the construction of history began: Heisenberg's version in his intellectual autobiography was that he knew the project was unviable in the short and medium term and, therefore, there was no risk in advancing the research. Following Benjamin's theses on the philosophy of history, I will brush against this construction of the history of science to bring to light several reasons that dismantle this construction. To do this I will resort to the philosophy of science of Popper, Kuhn and Lakatos and Marcuse's criticism of technological rationality. The aim is threefold: to question the vision of science held by Heisenberg, to show the influence of the great scientific-technological projects on the way of thinking and acting of individuals and to bridge the two abysses denounced by the physicist and novelist C. P. Snow, one between sciences and humanities and the other between those who have specialized knowledge and those who do not. (shrink)
